Welcome to Active Rehabilitation!

Screen Shot 2014-03-25 at 3.52.02 PMAt Active Rehabilitation, you will be working with an exceptional team of professionals and we encourage you to read about our individual therapists.  Our main objective is to get you to your full potential regarding function and pain. We also focus on giving you the best service possible.  Please feel free to ask us questions or give us advice about any possible improvements we can make.

First, we will examine the injury and determine the cause of it.  It is not good enough to treat the symptoms, we need to solve the cause of the problem.  There is great variety in causes possible (bad posture, weakness, tightness, wrong shoes, bad training methods, etc.). Based on the diagnosis, the cause, and the symptoms, we make a plan of action, your treatment plan.  We will explain your injury to you, the treatment plan, and what you can do to speed up the healing process.

Your first appointment will last about 1 hour.  Follow-up appointments are between 30 to 60 minutes depending on your injury and the therapist’s assessment of what is necessary.

To help you the most from the very beginning please do the following:

  1. Bring or wear comfortable clothing allowing you to move freely.
  2. Bring the results of any X-rays, MRI, or other tests relevant to your injury.
  3. Print out the forms below and fill them out before your first appointment.  Please don’t forget to bring it with you during the first visit!  If you are unable to do this, please arrive 20 minutes before your first appointment to fill out the forms.
  4. Bring your insurance card.

To schedule an appointment, call us at (626) 795-0800. Appointments can be made anytime from 7:00am to 7:00pm, Monday through Friday, and from 7:00am till 11:30am on Saturday. There is no charge for changing or cancelling your appointment as long as you give us at least 24 hours notice prior to your scheduled time.

We work with many health plans and insurance companies.  There are some insurance plans, however, that we are not able to work with and still maintain our services at a good level of care. Although most insurance companies do cover physical therapy, what they cover specifically varies greatly. When calling our office to make an appointment, we will call your insurance and find out your benefits. We will let you know what your insurance will cover and if you are responsible for a part of the cost. Whenever you have any questions regarding insurance, billing or payment options, please don’t hesitate to call us at (626) 795-0800.
